Inspire Issue Letters

Due to an error, you were not notified by Premera that your provider was leaving our network. These providers left the Premera network between January 1 and July 31, 2025. We are required by Washington State mandates to notify you of this change if you received care from a provider that terminated from our network.

With your provider being out of network, non-emergency care may not be covered or will cost more out of pocket if you receive care from an out of network provider or facility. If your provider returns to our network, they will be considered in-network and claims will be processed at the in-network benefit level.

If you received services from the provider noted in your letter after their termination date, Premera will reprocess your claim(s) to pay at the in-network benefit level. You will receive an updated Explanation of Benefits.

You can limit your out-of-pocket costs by seeing doctors in your plan network whenever possible.

To find a new in-network doctor who is right for you:

  • Sign into your account at <URL>
  • Select Find Care, then Find a Doctor
  • Search the provider directory to find a provider that meets your needs
  • You can also contact us at the number on your ID card for help in finding a new provider.
